after fire report

Recently with had a fire which resulted in the save of 3 children. We have a new fire chief who would like an "after fire report" completed on this fire. I assume he wants included info on the fire as well as any thoughts about personnel who should be considered for heroism awards. Has anyone completed something like this in the past & do you have a copy of such form available for our use?
