Just want to know if anyone out there has standing orders for pain control and if so what meds are you using.

The only standing pain control orders we have are for chest pain patients. Anything else we call in for, although the docs are generally more than willing to okay orders.

We can only give MSO4 for chest pain after nitro without calling in.
Our pain meds are:
1. MSO4
2. Demeral
3. Toradol
4. Nubain
5. Versed (Not really a pain med, but then again if you can't remember the pain it never happened) http://www.firehouse.com/interactive/boards/biggrin.gif
Running a code is easy,
The hard part is when you get them back and have a 30-40 min transport.

We are rather agrssive with pain management in our system. We can choose one of the folowing based on MOI.
1) Morphine
2) Statol
3) Nitus Oxide
4) Fentanyl
5) Versed
6) Valium
Our general all purpose narc is Fentanyl and Nitrus Oxide, we are able to use those together as well as promethazine with good results. The Fentanyl has a very short half like so the Doc's @ the ED dont have a big problem receiving a medicated pt.

WE USE MS AND NO2 FOR PAIN. UTILIZING THE PAIN SCALE (1 TO 10) WE DECIDE WHAT WE ARE GOING TO USE 1 TO 4 NITROUS 5 TO 10 MORPHINE. WE HAVEN'T HAD ANY PROBLEMS TO MY KNOWLEDGE WITH ABUSE OF PAIN MEDS.

We have a standing order for MSO4 for patients with CP and/or Pulmonary Edema. We also have it and Toradol on standing order for isolated extremity trauma. (MS up to 5 mg and then we must call for more)
The only time we have to call and request before initiating pain management is in our protocol titled Severe Pain Management. That's for things such as kidney stone pain.

We have standing orders for MS for CP after NTG if the pn is a 5 or greater. As for pain other than that, we have to get orders.
We carry:
MS, Valium & Stadol. Used to carry NO2...

We have an isolated pain protocol, using MS,and Demerol. We can give MS to chest pain patients without orders also. Some of our hospitals get mighty TICKED about us medicating some patients with nasty FX's and dislocations. The maximun is 5 mg of MS and 25mg's of Demerol.
